ClientTrack is a case management SaaS platform hosted in Microsoft Azure. Using ClientTrack as a central hub for case management helps break down data silos that are all too frequent in health and human services.

At its core, ClientTrack is a SQL database that enables organizations serving vulnerable populations to intake clients, assess their eligibility for needed services, and then refer them to those services. ClientTrack’s reporting tools allow organizations to gather client data and report to stakeholders on progress made.

ClientTrack is a web application with a web front end, middle tier, and data access layer. The front end of the application is the presentation tier that renders the user interface (UI) and takes user input. ClientTrack has a unique Forms Engine that uses configuration data to dynamically render the UI. This allows the application UI workflow as well as forms to be highly customizable based on the County’s specific business needs.
